Waiver of Liability
Participant hereby waives and releases the Retreat Centre & Mindful Switch, its employees, agents, volunteers, and any affiliated individuals or organizations from any and all claims, demands, or causes of action arising out of or related to any injury, loss, or damage sustained by the Participant during the retreat, whether caused by negligence, accident, or otherwise.
Health and Medical Conditions
Participant represents and warrants that they are in good physical and mental health and capable of participating in all activities involved in the retreat. Participant acknowledge that it is their responsibility to consult with a healthcare professional before engaging in any retreat activities if they have concerns about their health or physical condition. The Retreat Centre and Mindful Switch is not responsible for any medical expenses, injuries, or medical conditions that may arise.
Emergency Treatment: In the event of an emergency, Participant gives their consent for medical treatment, and Participant agrees to be responsible for any costs associated with such treatment. The Retreat Centre and Mindful Switch will make reasonable efforts to contact an emergency contact provided by the Participant in case of emergency.
Acknowledgment and Acceptance: By signing below, Participant acknowledges that they have read and fully understand this Waiver of Liability and Assumption of Risk Agreement, and that they voluntarily agree to the terms set forth.
Consent
I have read this agreement and fully understand its contents. I sign it of my own free will. I am of legal age and accept the above disclaimer and authorization.
